ADMISSION TO THE DORMITORY
Applicants are required to submit requirements for their application to be considered.
Documents to be submitted depending on employment status of parents:

*ITR or Income Tax Return – should be year 2025 and stamped received by the Bureau of Internal Revenue on 2026; ITR of previous years will not be accepted.
**Affidavit is a written document prepared and signed by a lawyer.
IMPORTANT REMINDERS
- Complete the form truthfully.
- Note that not all applicants can be accommodated in the dormitory.
- Only fully completed requirements will be considered for processing.
Remember: To be admitted to the dormitory is not a right. At most, it is only a privilege. As such, it may be revoked anytime subject to the requirements and policies of the University and the Dormitory. Hence, the Dormitory Manager enforce the rules and recommend appropriate action to the dormitory committee.

You must provide truthful, consistent, and accurate information in all forms and documents. Any mismatch, incorrect data, or dishonesty will lead to automatic disqualification.

| 4. Form 5 Submission | You may submit the UP Form 5 or a Temporary Form 5 when you move in. |
| 5. Incomplete Requirements | Applicants who qualify but lack documents cannot move in until complete. |
| 6. Evaluation Process | • The Dormitory Committee will conduct a fair and thorough review. • Final results will be released only after deliberation. |
